Indemnity & Information Form
All participants are required to complete an Indemnity & Information Form before the start of any tour. This form provides us with the necessary personal and medical details to ensure your safety, and it confirms your acknowledgment of the terms of participation.
Please view document, complete it in full, download(save) the form below, and then upload it using the submission form provided on our website.

For your convenience, we’ve partnered with PayFast to provide a quick, reliable, and secure online payment method. Please note that the Paying Account Merchant (PAM) will appear as Aardvark Adventures For You when completing your payment.
